Information Required For Your Estate Plan

In preparing your estate planning documents, we will need the following information.

  • Executor/Successor Trustee – The person or people who will be in charge of enforcing your wishes either through a Will or Trust. For a Successor Trustee, it is possible they will manage assets in your Trust for your benefit if you become incapacitated.
  • Beneficiaries & Distributions – The person or people to whom you would like your assets to go and in what dollar amounts or percentages.
  • Agent under Power of Attorney – The person or people who will manage your finances in the event of your temporary or permanent incapacity.
  • Health Care Surrogate – The person or people who will make health care decisions for you in the event of your temporary or permanent incapacity.

If you have minor children:

  • Guardian – The person or people who would act as guardian for your minor children.

For all those you will be appointing, we will need their full name (preferably as it appears on their driver’s license or other ID), address, best phone number, and relationship to you.  We will contact you if we need any further information.
